- Current (1yr or less) criminal background clearance
- Current (1yr or less) child abuse clearance
- Current (1yr or less) 2-step TB test or proof of positive TB test
- 2 written references preferably from past employers
- Clearance to work from your doctor
- Copy of state issued ID showing PA residency for 2 years
- If not a resident, FBI background check
- Proof of car insurance, if you drive
- Copies of trainings or credentials
